Post-Hospital and Recovery Care
Coming home is the risky part. We make it safe.
The first two weeks after a hospital stay are when seniors get readmitted. Someone skips a medication, tries the stairs alone, or does not eat because cooking is too much. Discharge planners at Torrance Memorial and Little Company of Mary know this, which is why they ask families who will be at home.

Quick answer
A Precious Elder provides post-hospital and post-surgery home care in the South Bay, including the ride home, meals, safe mobility, medication reminders and follow-up appointment support.
We can be the answer. A caregiver meets your parent at discharge, gets them home and settled, and covers the days or weeks it takes to get steady again. We coordinate with the home health nurse or physical therapist so nothing overlaps and nothing is missed.
What's included
- Transportation home from the hospital or rehab
- Setting up the home for recovery

- Short-term coverage, from a few days to a few weeks
Who this is for
- Hip, knee and back surgery recovery

- Discharges where the family lives far away
- Anyone the hospital will not release without help at home
Recovery Care questions
How fast can you start after a discharge?
Our goal is within 48 hours, and we do our best to start the same day after a discharge. Call us as soon as you know the discharge date.
Is this covered by Medicare?
Medicare covers skilled home health (nurses, therapists) for a limited period, not non-medical home care. Our care is private pay, long-term care insurance or VA funded. We work alongside the Medicare home health agency, not instead of it.
What if my father only needs help for a week?
That is fine. Short-term care has the same 4-hour minimum and no long-term contract. Many recovery clients use us for one to three weeks and stop when they are steady.
